Facebook ads vs. Google ads for clinics (2026): which one first, and why it depends on your treatment

Google catches people already searching. Meta reaches people who aren't yet. For clinics, which one to start with depends on how your patients decide.

"Should we do Facebook or Google?" is usually asked as if one is better. They do different jobs, and for a clinic the right starting point depends almost entirely on how your patients decide to book.

The one-line difference

Google captures demand. Someone types "botox near me." They already want it. You're competing to be chosen.

Meta creates demand. Someone scrolling Instagram wasn't looking for you. The ad has to make them want it — and the offer has to qualify them.

Google is the best channel for people who already decided. Meta is the best channel for people who haven't yet. Most aesthetic patients start in the second group.

How that plays out by treatment

Latent-demand treatments — injectables, skin treatments, body contouring, wellness, weight loss. Many patients have thought about it for months without searching. Meta reaches them before they search, which also means before they're comparing you against everyone on page one. Meta usually wins here.

Urgent or specific-intent services — "acne scar treatment [city]," "IV therapy near me today." People search the moment they decide. Google usually wins here.

High-consideration treatments — anything with a long decision cycle and a high price. Both matter: Meta starts the consideration, Google catches them when they're ready. This is where running both pays for itself.

The constraint both share

Both platforms restrict healthcare advertising, in different ways. On Meta, the Personal Attributes policy rejects copy implying the viewer has a condition, and before-and-after imagery is heavily restricted — here's what gets clinic ads rejected. And your state's professional advertising rules apply to both equally; we compared California, New York and Florida on our research page.

Which first

For most aesthetic and wellness clinics: Meta first. The demand is latent, the audience is far larger, and click costs are lower. It also tells you within weeks which offers and messages your market responds to.

For urgent, search-driven services: Google first. Be where people look the moment they decide.

Either way, build the local foundation underneath. Your Google Business Profile and reviews feed the local map results that sit above the ads for "near me" searches — and they keep producing after the ad budget stops. How to rank there without a storefront.
